On-Line Registration:

On-line registration is the preferred registration method. You can register using your Credit Card via PayPal.

Please read the waiver and fill out the online registration form further down on this page. You will then be taken to a payment page where you can pay your registration fees.

Register By Mail:

You can register your child by mail and pay by check or money-order. To register by mail, click on the link below and follow the step by step instructions.

Register In-Person:

You can register in person with your check, money order or cash (receipts will be issued on site). Please bring a check or money order with your confirmation number on the check or money order.

As the parent or legal guardian of the child named on the reverse side of this form, I, the undersigned, do hereby give my consent and approval of said child to participate in all DOTSON FAMILY PARK activities. I also agree to:

Provide an affidavit to allow free and unfettered access to birth records for verification of date of birth (if requested) and to allow school records to be checked to further prove that there was NO participation in the sport of football during the current season
Support highest standards of teamwork and good sportsmanship
Adhere to and support all DOTSON FAMILY PARK Rules and Regulations
Participate in the WITH DOTSON FAMILY PARK fund raising programs
Understand that if I choose the candy option I may NOT return the candy
Pay all fees due to DOTSON FAMILY PARK (fundraiser and/or registration) before the specified date
Allow DOTSON FAMILY PARK to use images without name identification for the purpose of promoting and/or marketing through the website, flyers or any other type of promotional material.
Return all rented football equipment to DOTSON FAMILY PARK by 12/31/2011, failure to return the equipment will in fact be theft of the equipment and should be considered as such.
If rented equipment is not returned by 12/31/2011 admit that not returning the equipment as requested should be considered theft and charges of theft should be filed with the Houston Police Department.

I also understand that there are NO REFUNDS of any fees without approval of the DOTSON FAMILY PARK Board of Directors. Only cases involving serious injury, debilitating disease or unforeseen relocation to an area outside a reasonable proximity to DOTSON FAMILY PARK, as determined by the DOTSON FAMILY PARK Board of Directors, will be reviewed for possible refund.
